How do you get the reduced price icon on a book when no price has been reduced?

That should not be possible, but perhaps the trick to do it is first raise the price and then lower it again to the original price.

Errors can occur. Such as entering a wrong price when issuing a book then correcting it (down). The icon will then appear indicating a price reduction, even if the book has not yet been activated. Tim is aware of the problem.
There are other ways to get it to show up which I will refrain from describing,, but if and when it is done deliberately, it is not allowed.
If you suspect that someone is gaming the software to get an illegal advantage, note that it is strictly prohibited. It could bring consequences, such as suspension from using the platform..
If it is your suspicion, then bring it to the attention of the moderator to look into. Rrr...

Is it possible that if you start the book with so many pages and later on you add extra pages will the icon show up then?

It is going to show whenever price is changed (lower) on a page. Old page or any added page. The key is reducing the price on a completed page. Often when you put a book together, or add pages later, a mistake can crop up in the process. One mistake that has happened to me from time to time, as an example, is entering 20 for 20cents instead of $0.20. It is then recorded as $20.00. When checking the book before activating it you will spot it. When you make a correction to put your intended lower price of $0.20, the icon will show up. The price reduction icon is visible for 3 days.
I have requested a software fix, but it is low on the priority list of Tim and may be too complex to program for the minor benefit it offers.

I've done exactly what Ralph described. I forgot the decimal point and my 10c price became $10.00. When I edited the book and inserted that decimal point it showed a price reduction. We all make mistakes now and then.

As Ralph stated, normally this is not a problem and not abused. If, however, a seller is using some method to gain a price reduction symbol on their books to increase views and it becomes obvious that they are doing so then they will risk being suspended.
